THE ADMISSIONS PROCESS

A structured pathway, explained at every stage

At High View Care Services, the process of joining us is not an administrative hurdle. It is the beginning of the clinical relationship. The care and attention we give to the pre-admission process is a direct reflection of how we work once a client is with us.

Our pathway has six stages, from initial referral through to discharge planning. Each stage has a clear purpose, clear timescales and clear communication with the client, their family and the professionals involved. No one should be left wondering what happens next.

Our pathway is designed around three placement options, ensuring that support is tailored to each individual's needs, goals and anticipated length of rehabilitation.

Short-Term Placements (STP) typically last between 3 and 6 months and are suited to individuals with clearly defined rehabilitation goals and an identified pathway towards discharge or progression.

Medium-Term Placements (MTP) are designed for individuals who require a longer period of structured rehabilitation and specialist support to achieve their goals and maximise independence.

Long-Term Placements (LTP) provide ongoing rehabilitation and support for individuals with more complex needs who would benefit from a substantially extended period of specialist intervention and structured care.

As part of our pre-admission assessment process, we work closely with individuals, families and professionals to determine the most appropriate placement option and pathway.

OUR PATHWAY

The six stages of Admission

Stage 1

Initial Referral

The process begins with your initial enquiry, which can be made by phone, email or via our online referral form. At this stage, we will ask for background information about the person, their current situation, their diagnosis and the proposed placement duration. Within two working days of receiving a referral, our Assessment Team will carry out a preliminary review of suitability and coordinate the assessment process.

Stage 2

Pre-Admission Assessment

If the preliminary review indicates that High View Care Services is an appropriate service, we move toward the pre-admission assessment. This is a collaborative process involving the client, their family or important others and the referring professional, wherever possible in the client's current setting. Together, we explore the person's strengths, needs and goals and build a clear picture of what support will look like and provide transparency of the services we provide. At the end of this stage, we produce a letter of outcome confirming proposed placement with weekly costings. Where funding has been agreed, a full placement proposal will be provided, including the recommended support provision and the client's goals.

Stage 3

Transition

Before admission, we work closely with the individual, their family and professionals to plan a safe and successful transition. Wherever possible, individuals are invited to visit the service, meet the team and become familiar with their future home. Through a collaborative pre-transition process, we agree admission arrangements, review support needs and ensure all relevant information is up to date, helping to create a smooth and positive start to their journey with us.

Stage 4

Admission

Admission is focused on helping the individual feel welcomed, settled and understood. During this stage, we take time to build rapport, get to know the person beyond their diagnosis, and understand what matters most to them, their strengths, aspirations and support needs. A dedicated key worker is allocated to help develop a trusting relationship and provide a consistent point of contact throughout their journey.

Alongside this, we establish baseline measures of cognitive, emotional and functional abilities, providing the foundation for personalised planning, goal setting and outcome measurement.

Stage 5

Monitoring Outcomes

At the 6 week point a formal review is held with the client and their stakeholders. Baseline assessments are reviewed in detail, timescales and resource requirements are revisited. We continue to monitor the clients progress and outcomes within High View Care Services and this review shapes the next phase of the client's plan.

Stage 6

Discharge

Discharge planning begins well before an individual leaves our service, ensuring that progress is sustained and future support is carefully considered. Working collaboratively with the individual, their family and professionals, we develop a personalised transition plan that supports the next stage of their journey.

Where an individual is moving to a new service or living arrangement, we work closely with providers to ensure a smooth and coordinated handover. A comprehensive discharge report is completed, summarising progress, outcomes and recommendations, helping to support ongoing success beyond High View Care Services.

For individuals who would benefit from continued specialist input, our community outreach service can provide ongoing rehabilitation and support within their own home and local community.

Clear, honest communication throughout

We know that placing a family member in specialist residential care involves a great deal of trust. We take that responsibility seriously.

From the moment you first get in touch, we will keep you informed at every stage. During the pre-admission assessment, we will involve you in building the picture of who your family member is and what they need. During the placement, you will receive regular updates on how things are going, what the plan looks like and what comes next. You will always have a direct point of contact in the team.

Our approach is open, transparent and person-centred. We communicate honestly and work in partnership with those around the individual to ensure expectations remain aligned and the focus remains on achieving the best possible outcomes.
