Our therapy-led Outreach Service provides specialist neurorehabilitation and wellbeing support to adults with acquired brain injury, mental health needs, co-morbidities and other complex needs within their own homes and communities.

What Is Our Outreach Service?

Our Outreach Service, also known as HVON (High View Outreach Neurorehabilitation), brings the same specialist rehabilitation and wellbeing support we are known for out of our residential homes and into the community. It is built for adults living with the effects of acquired brain injury, mental health needs, co-occurring conditions and other complex needs, who want to build their independence in their own home and community.

Every person we support receives an individual assessment and a support plan shaped entirely around their goals. That plan is delivered by our specialist Neuro Rehabilitation Coaches, working under the guidance of our Trans-disciplinary Team (TDT), the same group of Clinical Psychologists, Occupational Therapists, Social Workers and other specialists who guide care across all of our services.

Support can include:

  • Developing independent living skills
  • Building safe access to the community
  • Cognitive and behavioural rehabilitation
  • Emotional wellbeing and relapse prevention
  • Establishing meaningful routines, activities and relationships
  • Supporting progress towards education, volunteering or employment

Outreach Services

Who It's For

Our Outreach Service is for adults managing acquired brain injury, mental health difficulties, substance misuse histories or other complex needs, who are ready to build their independence outside of a residential setting. Some clients move to us from one of our residential rehabilitation services. Others are referred to us directly and never need a residential placement at all.

Whatever someone's history, our starting point stays the same. A genuine belief in what's possible for them, delivered by a team who know how to turn that belief into everyday progress.

Our four specialist homes across South East London offer structured, therapeutic environments for adults with acquired brain injury, mental health, comorbidities and complex needs. Each one is calm, purposeful and focused on real progress and stability.

Outreach Services

A Flexible Pathway

We built our Outreach Service to flex around each person, not the other way around.

Transitional support: clients can move from one of our residential rehabilitation services into our Transitional Living Unit, then on into their own home with ongoing outreach support. This gives people time to build confidence and skills gradually, with our team alongside them at every stage.

Direct referral: clients already living at home can be referred straight to our Outreach Service, without ever needing to access residential care.

Whichever route someone takes, we offer both short-term rehabilitation programmes and longer-term support, depending on individual needs and goals. We work collaboratively with families, case managers, commissioners and other professionals throughout, with regular reviews to monitor progress and adapt support as circumstances change.

Making a Referral

Referrals can be made by case managers, Local Authorities, ICBs (Integrated Care Boards), hospitals, solicitors, deputies, other care providers, families or individuals themselves.

The important questions

Do clients need to have stayed at a High View residential service first?

No. While some clients transition to us from one of our residential rehabilitation services, many are referred to our Outreach Service directly and never need a residential placement.

How often is progress reviewed?

We review progress regularly with the client, their family and any other professionals involved, adjusting the support plan as needs and goals change.

How is the Outreach Service funded?

Funding arrangements vary depending on the individual and the referring body. Visit our Funding, Fees and Referral page for more detail, or speak to our team directly.

Can the level of support change over time?

Yes. Support can increase or decrease as a person's needs change, moving between short-term rehabilitation and longer-term wellbeing support as required.
